ABOUT THE SERVICE:

      Certified Transcript or photocopy of civil registry documents like birth (Form 1A) marriage (Form 3A) and death certificates (Form 2A) may be secured from the City Civil Registrar’s Office. These documents are usually needed for enrolment, driver’s license and passport application, employment, or to establish the occurrence of an event.

●        Latest Community Tax Certificate

●        Official receipt for the fees

●        Authorization Letter (if not the concerned person)

●        Valid ID

2.    Abroad            -P 400.00

Return to the Civil Registrar’s Office

-present official receipt to the employee in-charge.

  1. The certificate is computerized and printed by the EIC
  2. The Civil Registrar reviews and signs the certificate

 

-Computerize and print the certificate

-get the true copy of the certificate

How to file complaint?

The employee concerned will be called to a meeting  to hear  his side and the complainant will be inform the result

How complaints are processed?

Formal complaint address to the local Civil Registrar or the City Administrator stating the incident evidence and the employee complained of.
