Not less than 2 public or private documents provided by the applicant upon which the correction shall be based. Examples of these documents are the following:
a. baptismal certificate,
b. voter's affidavit,
c. employment record,
d. GSIS/SSS record,
e. medical record,
f. school record,
g. business record,
h. driver's license,
i. insurance,
j. land titles,
k. certificate of land transfer,
l. bank passbook,
m. NBI/police clearance,
n. civil registry records of ascendants, and
o. other documents which the petitioner or the city or municipal civil registrar or the consul general may consider relevant and necessary for the approval of the petition.
