Delayed Registration of Death (Occurrence - At Home)


The Certificate of Death must be registered within 30-day reglementary period in Civil Registry office of the place where the death occurred. Death which is considered a Medico-Legal Case-death which is under investigation by proper investigative body of the government autopsy or examination by medico legal officers.

CHECKLIST OF REQUIREMENTS

Documents provided by the Applicant: 

  1. Four (4) copies of Certificate of Death, which must be accomplished correctly and completely;    
  2. Affidavit for delayed registration which shall be executed by the hospital or clinic administrator if the person died in a hospital, clinic or in a similar institution, or by the attendant at death if the person died elsewhere. In default of the hospital or clinic administrator or attendant at death, the affidavit  shall be executed by any of the nearest relative of the deceased, or by any person having legal charge of the deceased when the latter was still alive;
  3. The affidavit referred to shall state among other things, the name of the deceased, the facts of his death, the date and place of burial or cremation, and the circumstances why the death was not reported for registration within thirty (30) days after death;
  4. Authenticated copy of the certificate of burial, cremation, or of other means of corpse disposal; and 
  5. Approval for registration by the health officer in the box provided in the Certificate of Death.

1 Client Steps:  Go to the Public Assistance and Complaints Desk (PACD) to avail of the service.    
Agency Action: Requires client to sign the logbook and advise him / her to proceed to the person in-charge of the service for submission of requirements.    ​​​​​​​

2 Client Steps:  Proceed to the employee in-charge and submit the documents as required.    ​​​​​​​
Agency Action: Receive and review the documents, conducts interview and issue order of payment to be paid at the office collector.    ​​​​​​​

3 Client Steps:  Present the order of payment and pay the corresponding fee at the office collector.    ​​​​​​​
Agency Action: Receive payment, issue Official Receipt and advise client to give it to the employee in-charge.    ​​​​​​​

4 Client Steps: Present the Official Receipt to the employee in-charge.    ​​​​​​​
Agency Action: Received and fill in Official Receipt’s details to the Certificate of Death, encode the data/documents and advise the informant to present the document to the City Health Officer for review.    ​​​​​​​

5 Client Steps: Present the documents to the City Health Officer​​​​​​​
Agency Action: City Health Officer identifies the cause of death and thereafter, if the dead was embalmed advise the client to proceed to embalmer for signature, otherwise back to City Civil Registrar’s Office for signature.     ​​​​​​​

6 Client Steps: Returns to the office of the civil registrar and present the document for his/her signature.​​​​​​​
Agency Action: Review and sign the documents ready for posting.    ​​​​

6.1 Agency Action: Prepares Notice of Posting and post it to the bulletin board for 10 days and advise the client to return after the lapse of the posting period.    ​​​​​​​Fees to be Paid: None    ​​​​​​​

6.2 Agency Action:  Assign registry number and enter it in the Civil Registry Book.    ​​​

7 Client Steps: Returns and claims the signed Death Certificate at the releasing section.    ​​​​​​​
