The certificate of marriage must be registered in the Civil Registry Office of the city where the marriage was solemnized within 15 days thereafter (30 days for marriages of exceptional character).

- Certificate of Marriage duly signed by the contracting parties, solemnizing officer and witnesses (4 original copies) taken from the Church, Courts of Justice, Office of the City Mayor where marriage is celebrated
- For marriage of exceptional character accompanied by an affidavit under article 34 of the civil code (4 original copies) taken from the City Civil Registrar’s Office / Lawyer / Public Attorney’s Office (PAO)
- Official Receipt (OR) for Processing Fee (1 original copy) taken from the City Civil Registrar’s Office Collector
- Affidavit of delayed registration executed by the administrator/church’s in charge or the couple itself (1 original and 1 photocopy) taken from the Lawyers, etc.
