  1. Certified copy of the Court Decision (2 original copies) from the Courts of Justice
  2. Certificate copy of Entry of Final Judgment (2 original copies) from the Courts of Justice
  3. Certificate of Live Birth of the Child (2 original copies) from the Applicant / City Civil Registrar’s Office
  4. Certificate of Marriage of the adoptee (2 original copies) from the Applicant / City Civil Registrar’s Office
Steps Service Details
1 Client Steps:   Go to the  Public Assistance and Complaints Desk (PACD) to avail of the service.    
Agency Action:  Requires client to sign the logbook and advise him / her to proceed to the person in-charge of the service for submission of requirements.

2 Client Steps:   Proceed to the employee in-charge and submit the documents as required.​​​​​​​​​​​​​​
Agency Action:  Receive and review the documents, record in the Registry of Book of Court Decree and issue order of payment to be paid at the office collector.      ​​​​​​​

Person Responsible:  Registration Officer II from the City Civil Registrar’s Office
3 Client Steps:   Present the order of payment and pay the corresponding fee at the office collector.    ​​​​​​​​​​​​​​
Agency Action:  Receive payment, issue official receipt and advise client to give it to the employee in-charge.​​​​​​​​​​​​​​
Fees to be Paid:  PHP 1,000.00 for Local; PHP 2,000.00 for Foreigner     ​​​​​​​​​​​​​​

Person Responsible:  Bonded Collector from the City Civil Registrar’s Office
4 Client Steps:   Present the Official Receipt to the employee in-charge.    ​​​​​​​​​​​​​​
Agency Action:  Received the official receipt, encode and print the Certificate of Authenticity and the amended Birth Certificate for the adoptee signature     ​​​​​​​​​​​​​​

5 Client Steps:   Receive and sign the amended Certificate  of Authenticity and of the amended Birth Certificate  and give it back to the employee in charge for the signature of the City Civil Registrar    ​​​​​​​​​​​​​​
Agency Action:  Civil Registrar signs the Certificate of Authenticity and the amended Birth Certificate for SECPA issuance from PSA.    ​​​​​​​​​​​​​​

5.1 Agency Action: ​​​​​​​ Prepared the transmittal and forwards the signed documents to PSA, Manila for issuance of SECPA for 2 months and advise client to return after the lapse of 2 months period to the CCRO.    ​​​​​​​​​​​​​​​​​​​​​

Agency Action:  Release the issued SECPA and requires the client to sign the logbook.  ​​​​​​​​​​​​​​
