1 Client Steps:   Go to the Public Assistance and Complaints Desk (PACD) to avail of the service.    
Agency Action:  Requires client to sign the logbook and advise him / her to proceed to the person in-charge of the service for submission of requirements.

2 Client Steps:   Proceed to the employee in-charge and submit the documents as required.    ​​​​​​​​​​​​​​
Agency Action:  Receive and review the court decision and the entry of final judgment, record in the Registry Book of Court Decree and issue order of payment to be paid at the office collector.      ​​​​​​​​​​​​​​

3 Client Steps:   Present the order of payment and pay the corresponding fee at the office collector.    ​​​​​​​​​​​​​​
Agency Action:  Receive payment, issue official receipt and advise client to give it to the employee in-charge.    ​​​​​​​​​​​​​​

4 Client Steps:   Present the Official Receipt to the employee in-charge.    ​​​​​​​​​​​​​​
Agency Action:  Received the official receipt, encode and print the Certification of Court Decree and the Certificate of Authenticity for Civil Registrar signature.    ​​​​​​​

4.1 Agency Action:  The Civil Registrar signs the Certification of Court Decree and the Certificate of Authenticity.    ​​​​​​​​​​​​​​

4.2 Agency Action:  Forwards the signed documents to PSA, Manila for SECPA annotation and advises client to return after the lapse of 3 months period to the CCRO.    ​​​​​​​

5.2 Client Steps:   Returns and claims the Annotated SECPA at the releasing section.    ​​​​​​​​​​​​​​
Agency Action:  Release the Annotated SECPA and requires the client to sign the logbook.      ​​​​​​​​​​​​​​
