  1. Registered Certificate of Marriage of Parents (1 original)    from the City Civil Registrar’s Office / Owner’s copy / Philippine Statistics Authority (PSA)
  2. Certificate of Live Birth of the Child (1 original) from the City Civil Registrar’s Office / Owner’s copy
  3. Affidavit of Legitimation executed by both parents from the City Civil Registrar’s Office / Public Attorney’s
  4. Certification of registration of legal instrument (Affidavit of Legitimation) from the City Civil Registrar’s Office / Public Attorney’s
Steps Service Details
1 Client Steps:    Go to the Public Assistance and Complaints Desk (PACD) to avail of the service.    
Agency Action: Requires client to sign the logbook and advise him / her to proceed to the person in-charge of the service for submission of requirements.

2 Client Steps:    Proceed to the employee in-charge and submit the documents as required.    
Agency Action:   Receive and review the documents, conducts interview, encode and print the Affidavit of Legitimation ​​​​​​​​​​​​​​

2.1 Agency Action:   Let the informant check the entries and the parents acknowledge the same by signing the printed Form of Affidavit of Legitimation
3 Client Steps: Check the entries, acknowledge by signing the Affidavit of Legitimation and give it back to the employee in-charge for registration in the Register Book of Legal Instruments     ​​​​​​​​​​​​​​
Agency Action:   Receive and register the Affidavit of Legitimation in the Register Book of Legal Instruments and issue order of payment to be paid at the office collector.    ​​​​​​​

4 Client Steps:    Present the order of payment and pay the corresponding fee at the office collector.    ​​​​​​​​​​​​​​
Agency Action:   Receive payment, issue official receipt and advise client to give it to the employee in-charge.    ​​​​​​​​​​​​​​

Person Responsible:  Bonded Collector from the City Civil Registrar’s Office
5.  Client Steps:    Present the official receipt to the employee in-charge.    
Agency Action:   Receive and record the official receipt, make transmittal and proper corresponding formal annotation of the Certificate of Live Birth ready to be forward to PSA, Manila.

5.1 Agency Action:   Forward the document to PSA, Manila     ​​​​​​​​​​​​​​

5.2 Agency Action:   After 3 months the employee in-charge will notify the client to claim the annotated documents    ​​​​​​​

6 Client Steps:     Returns and claims the Annotated SECPA at the releasing section.    ​​​​​​​​​​​​​​
Agency Action:   Release the Annotated SECPA and requires the client to sign the logbook.      ​​​​​​​​​​​​​​
