Timely Registration of Death Occurred at Home


The Certificate of Death shall be registered at the Local Civil Registry Office where the death occurred within (30) days. If the deceased died without medical attendance, The City Health Officer shall examine the deceased certify as to cause of death.

CHECKLIST OF REQUIREMENTS

  1. Birth Certificate or Baptismal of the Deceased (1 original and 1 photocopy) from the City Civil Registrar’s copy / Personal / Philippine Statistics Authority (PSA)
  2. Barangay Certification (2 original copies) from the Barangay Office
Steps Service Details
1 Client Steps:    Go to the Public Assistance and Complaints Desk (PACD) to avail of the service.    
Agency Action:  Requires client to sign the logbook and advise him / her to proceed to the person in-charge of the service for submission of requirements.

2 Client Steps:   Proceed to the employee in-charge and submit the documents as required.    
Agency Action:  Receive the documents, interviews the client/informant and issue order of payment to be paid at the office collector.

Processign Time:  5 Minutes    
Person Responsible: Registration Officer II from the City Civil Registrar’s Office
3 Client Steps:   Present the order of payment and pay the corresponding fee at the office collector.    
Agency Action:  Receive payment, issue official receipt and advise client to give it to the employee in-charge.    ​​​​​​​

4 Client Steps:   Return to the employee in-charge and present the official receipt.    ​​​​​​​
Agency Action:  Receive and record the official receipt; encode and print the Form No. 103 Certificate of Death;     ​​​​​​​

4.1 Agency Action:  Let the informant sign the documents and advise him/her to proceed to City Health Office for death diagnoses.    ​​​​​​​​​​​​​​
5 Client Steps:  Sign and present the documents to the City Health Officer for identification of the cause of death.      ​​​​​​​​​​​​​​
Agency Action:  ​​​​​​​​​​​​​​City Health Officer identifies the cause of death and thereafter, if the dead was embalmed advises client to proceed to the embalmer  for his signature and return to the office of CCRO.    ​​​​​​​

6 Client Steps:   Returns to the office of the civil registrar and present the document for his/her signature         ​​​​​​​​​​​​​​

7 Client Steps: Claims the signed Form 103 (Certificate of Death) at the releasing section.       ​​​​​​​​​​​​​​
Agency Action:  Release the signed Form 103 (Certificate of Death) and requires the client to sign the logbook.        ​​​​​​​​​​​​​​
