Accreditation of Cooperatives/Non-Government’s Organization (NGO’s) / People’s Organization (PO’s) / Civil Society Organization (CSO’s)

 The Local Government Unit is mandated to promote the establishment and operation of NON-GOVERNMENT’S ORGANIZATION (NGO’s)/ PEOPLE’S ORGANIZATION (PO’s)/ CIVIL SOCIETY ORGANIZATION (CSO’s), to make them active partners in the pursuit of local autonomy, and to directly involve them in the plans, programs, projects or activities of the LGU as well as the involvement in the delivery of basic services and facilities.

Classification: Special Law: Section 3 (d)., Rule VII  of the Implementing Rules and Regulations (IRR) of RA 11032 otherwise known as Ease of Doing Business and Efficient Government Service Delivery Act of 2018, states that “If the application or request for license, clearance, permit, certification or authorization shall require the approval of the Local Sangguniang  Bayan,  Sangguniang Panlungsod, or the Sangguniang Panlalawigan, as the case may be, the Sanggunian concerned shall be given a period of forty-five (45) working days to act on the application or request, which can be extended for another twenty (20) working days.  If the local Sanggunian concerned has denied the application or request, the reason for the denial, as well as the remedial measures that may be taken by the applicant shall be cited by the concerned Sanggunian.

CHECKLIST OF REQUIREMENTS
1. Sample Letter of Application from the SP Office.

2. Sample Application Form for Accreditation from the SP Office.

3. Certificate of Registration or Certificate of Accreditation (or in the case of IPO’s, certification issued by NCIP) from the National Commission on Indigenous Peoples (NCIP); or other agencies relevant to the applicants organization like Cooperative Development Authority (CDA); Securities and Exchange Commission (SEC); Department of Labor and Employment (DOLE) among others.

4. List of current officer and members from the Applicant.

5. Constitution and By-Laws from the Applicant.

6. Certification from the Barangay that the applicant established an office in the city from the concerned Barangay.

7. Business Permit, if applicable from the concerned Barangay.

8. For existing CSOs, Minute of the Annual Meetings of the immediately preceding year as certified by the organizations’ board secretary from the applicant.

9. For existing CSOs, Annual Accomplishment Report for the immediately preceding year from the application.

10. For existing CSOs, Financial Statement, at the minimum, signed by the executive officers of the organization, also of the immediately preceding year, and indicating therein other information such as the source(s) of funds from the applicant.

11. Duly approved Board Resolution signifying intention for accreditation for the purpose of representation in the local special body from the applicant.

12. Original Sworn Statement, issued by the chief executive officer or equivalent officer of the CSO, stating that the CSO is an independent, non-partisan organization and that it will retain its autonomy while pursuing the advancement of the people’s interest through its membership in a local special body, after satisfying all the requirements and after securing a Certificate of Accreditation from the concerned Sanggunian from the applicant.

13. For CSOs applying to be members of the Local School or Health Boards, photocopy of profiles of at least three (3) individuals in the organization that will verify their involvement in education or health services, as prescribed in Section 98 and 102 of RA 7160 from the Applicant.

14. Attendance during the committee meeting scheduled for the review and scrutiny of their application  from the SP Officer.

15. Official Receipt for the require fees from the City Treasurer's Office.

Steps Service Details
1 Client Steps: Sign in the client logbook in the Public Assistance and Complaints Desk Officer.
Agency Action:

Give the logbook to the client for the attendance. Direct or accompany the client to the staff in charge of the service needed.

Person Responsible: Administrative Assisstant III of the City Treasurer's Office.

Client Steps: Proceed to the concerned staff for the submission of the documents to be reviewed.

 

Agency Action: Discuss with the client. If the required documents are complete, received and include it in the Calendar of Business of the Session. If the Documents are lacking, advise the client to comply it. Once the documents are ready, return to the office to include it in the Agenda of the Session.

3 Client Steps: Wait while.
Agency Action: Process the documents
Fees to be Paid: None
Processing Time: 44 Working Days, 15 Minutes
Person Responsible: Local Legislative Staff Officer III/ SP Secretary/ SP Members/ Administrative Officer I of the City Council's Office.
4 Client Steps: After receiving the information of approval from the CCO, pay the corresponding fee at the City Treasurer’s Office (CTO).
Agency Action: None
Fees to be Paid: Applying for accreditation (NEW) is Php. 300.00. Renewal of Accreditation is Php. 200.00.
Processing Time: 15 Minutes
Person Responsible:  Local Treasury Operations Officer V of the City Treasurer's Office.
5 Client Steps: Return to the City Council's Office (CCO). Present the Official receipt (OR). Receive the Accreditation documents. Sign the office file of the transmittal letter.
Agency Action: Attaint the needs of the client. Received and record the O.R. in the Logbook. Give the Accreditation documents and O.R.. Request client to sign the transmittal letter.
