Issuance of Motorized Tricycle Operator’s Permit (MTOP) Provisional Franchise
All owners of motorized tricycles are required to secure a franchise to operate or the Motorized Tricycle Operator‘s Permit (MTOP) under City Ordinance No. 2015-08-028.

CHECKLIST OF REQUIREMENTS
- Filled out Application Form ( 1 original copy) from the City Planning and Development Office cubicle 12
- Notarized authorization from the applicant, if filed by a representative.(only within 4th civil degree by consanguinity is allowed) (1 original copy) from the Applicant
- Certificate of Registration in the applicant‘s name-bearing ―with sidecar‖ mark (2 photocopies) from the Land Transportation Office/ Applicant‘s file
- Valid Official Receipt (LTO registration) in the applicant‘s name ( 1 original and 1 photocopy) from the Land Transportation Office/ Applicant‘s file
- Barangay Clearance stating that the applicant is a bonafide resident of Tangub City(2 photocopies) from the Barangay Hall (applicant‘s place of residence)
- Accident Insurance Policy (TPL&PL) 1 original and 1 photocopy from the Insurance Company
- Certificate of training on traffic rules and regulations. from the Tangub City Traffic Management Bureau (TCTMB) / Keepers for Peace and Accident Rescue Team (KPART), Tangub City
