Issuance of Motorized Tricycle Operator’s Permit (MTOP) Provisional Franchise


All owners of motorized tricycles are required to secure a franchise to operate or the Motorized Tricycle Operator‘s Permit (MTOP) under City Ordinance No. 2015-08-028.

CHECKLIST OF REQUIREMENTS

  1. Filled out Application Form ( 1 original copy) from the City Planning and Development Office cubicle 12
  2. Notarized authorization from the applicant, if filed by a representative.(only within 4th civil degree by consanguinity is allowed) (1 original copy) from the Applicant
  3. Certificate of Registration in the applicant‘s name-bearing ―with sidecar‖ mark (2 photocopies) from the Land Transportation Office/ Applicant‘s file
  4. Valid Official Receipt (LTO registration) in the applicant‘s name ( 1 original and 1 photocopy) from the Land Transportation Office/ Applicant‘s file
  5. Barangay Clearance stating that the applicant is a bonafide resident of Tangub City(2 photocopies) from the Barangay Hall (applicant‘s place of residence)
  6. Accident Insurance Policy (TPL&PL) 1 original and 1 photocopy from the Insurance Company
  7. Certificate of training on traffic rules and regulations. from the Tangub City Traffic Management Bureau (TCTMB) / Keepers for Peace and Accident Rescue Team (KPART), Tangub City

1 Client Steps: Sign in the Client Log Book at PACD
Agency Action: Give the log book to the client and instruct him/her to proceed to cubicle 12​​​​​​​

2.1 Client Steps: Accompany CPDO personnel in inspecting the unit/motorcab​​​​​​​
Agency Action: Conduct unit inspection​​​​​​​

2.2 Agency Action: Issue the order of payment to client if all required documents were given and the unit inspected. Advise the client to pay at the CTO​​​​​​​

3 Client Steps: Pay the required fees at the City Treasury Office (CTO) by showing the Order of Payment.​​​​​​​
Agency Action: Accept the payment based on the Order of Payment issued and give the O.R. to the client​​​​​​​

4 Client Steps: Return to City Planning and Development Office for processing and release of Provisional Franchise​​​​​​​
Agency Action: Check the official receipt , process the application & prepare the Provisional Franchise then endorse the same to the City Council‘s Office​​​​​​​

4.1 Agency Action: Receive and record the application and sign the MTOP Provisional Franchise​​​​​​​

4.2 Agency Action: Release the MTOP Provisional Franchise and instruct client to sign the logbook​​​​​​​

5 Client Steps: Receive the MTOP Provisional Franchise and sign the logbook​​​​​​​
Agency Action: Give the logbook to client for signature​​​​​​​: The Motorized Tricycle Operators‘ Permit (MTOP) will be issued once the Sangguniang Panlungsod grants the same in accordance with the Internal Rules of Procedure set pursuant to RA 7160​​​​​​​​​​​​​​
