CHECKLIST OF REQUIREMENTS
1. Birth Certificate (1 original and 1 photocopy) from the Client or City Civil Registrar Office

if the birth certificate is not available either:

  • Baptismal Certificate (1 original and 2 photocopies) from the Client or City Civil Registrar Office
  • COMELEC Certification showing his/her birthdate (1 original and 2 photocopies) from the COMELEC Office
Steps Service Details
1 Client Steps: Go to the CSWDO, approach the PACD and present the requirements to avail of the service.
Agency Action: PACD examine the completeness of the required documents and endorse the client to the focal person on senior citizen for an interview and filling up of the Application Form

2 Client Steps: Proceed to the Social Worker in charge of the interview filling out the registration form.
Agency Action: Conduct client interview, give Application Form for filling up by the client
