Re-issuance will be charged to students who lost their original copies of study load, ledger, school ID, temporary ID, clearance, permit and grade slip.

2 Client Steps: Pay the payable amount *make sure to secure official receipt
Agency Action: Receive the paymen
Fees to be Paid: Amount as assessed
Processing Time: 15 Minutes

2.1 Agency Action: Issue official receipt
Person Responsible: Administrative Aide II from the Cash Office
3 Client Steps: Claim the official receipt of payment
Agency Action: Entry payments to the system

4 Client Steps: Claim the document being requested. For School ID, proceed to the City Hall, near GSIS kiosk (Left Side)
Agency Action: Print and issue the requested document except the school ID
