Issuance of Certificate of Good Moral Character
The Certificate of Good Moral Character is issued by the Guidance and Admission Office in order to affirm that a former student/enrollee or alumnus/alumna has shown exemplary behavior during the time of his/her enrolment in the college. This certificate is usually requested by alumni and former students of GADTC.

CHECKLIST OF REQUIREMENTS
1. Request Form (REG-Form-013) with complete signatories from the Registrar’s Office.

2. Receipt of Payment from the Cash Office.

1 Client Steps: Present accomplished REG-Form-013 (Application for School Records) from the Registrar’s Office and Receipt of Payment​​​​​​​
Agency Action: Receive and check for the completeness of the Form and the Receipt of Payment. Issue Good Moral Request Slip​​​​​​​.

Processing Time: 2 Minutes
Person Responsible: Guidance Coordinator Guidance and Admission Office
2 Client Steps: Fill up and submit Good Moral request slip​​​​​​​
Agency Action: Check the correctness of entries in the Request Slip. Prepare the Good Moral Certification. Affix signature on the certification​​​​​​​
Fees to be Paid: None
Processing Time: 18 Minutes

2 Client Steps: Sign the Logbook for release of the Certification​​​​​​​
Agency Action: Give the logbook and release the Certification. File the REG-Form-013 (when Certificate of Good Moral Character is requested only).​​​​​​​
