  1. Incidental Report from the Student
  2. Accomplished Complaint Form from the OSA
  3. Student ID for Certificate of Registration from the Student

1 Client Steps: Present the Certificate of Registration or Student ID to secure Complaint Form
Agency Action: Provide the Complaint Form

Person Responsible: Instructor I from the Office of Student Affairs
2 Client Steps: Submit accomplished Complaint Form and duly signed Incidental Report.
Agency Action:  Receive and assess/check the documents presented

2.1 Agency Action: Give the logbook

3 Client Steps: Sign the logbook
Agency Action: Advise the complainant to wait for 72 hours for the respondent’s reply. (based on Student Manual)
