Assistance for Lost Item


This is a student service that provides assistance to Alfonsos who had lost item(s). This is to ensure welfare of GADTC community are catered by the College.

CHECKLIST OF REQUIREMENTS

  1. Lost Form from the Office of Student Affairs
  2. Student ID or Certificate of Registration from the Student

1 Client Steps: Report to the Office of Student Affairs-Prefect of Discipline and Formation Office and present the Student ID or Certificate of Registration
Agency Action: Receive the ID or COR and provide the LOST Form

Person Responsible: Instructor I from the Office of Student Affairs
2 Client Steps: Fill out the LOST Form and submit to the OSA
Agency Action: Receive and check the completeness of entries of the LOST Form

2.1 Agency Action: Check the Found record if the item has been reported.

Processing Time: 4 Minutes

2.2 Agency Action: Post the LOST Form in the OSA Bulletin Board if the item has not been reported otherwise inform the student if there’s any returned item/s that matched to the lost item.

3 Client Steps: Come back when notified (if item has not been found) and/or provide required data or information (if the item has been reported).

3.1 Agency Action: If the data provided of the lost item matched to the found item, give the logbook and release the item(s)
