Availment of Alfonsos’ Scholarship Program


This service is granted to students enrolled in Gov. Alfonso D. Tan College who excelled academically including non-academic scholarships or grants for those who have distinct talent or has contributory community work for the City Government of Tangub, province, region or in national level. A program supported including budgetary allocation by the Local Government Unit of Tangub. Student-applicant is screened based on the guidelines and number of approved grant set by the Institution.

CHECKLIST OF REQUIREMENTS
Academic Scholarship

  1. Academic Scholarship Application Form from the Office of Student Affairs (OSA)
  2. Grade slip(s) for two (2) consecutive semesters of the preceding Academic Year with no grade below 87 from the Registrar's Office
  3. Certificate of General Weighted Average for two (2) consecutive semesters of the preceding Academic Year with at least 91% and above duly signed by the College Registrar from the Registrar's Office
  4. Certificate of Good Moral Character duly signed by the Prefect of Student Discipline and Formation or College Guidance Counselor from the Guidance and Admission Office
  5. One (1) original copy of Barangay Certification of Residency from the Respective Barangay Office where applicant resides

Entrance Scholarship

  1. Academic Scholarship Application Form from the Office of Student Affairs
  2. Certificate as Class Valedictorian duly signed by the School Principal from where the applicant graduated High School
  3. Certificate of General Weighted Average duly signed by the School Registrar from where the applicant graduated High School
  4. Certificate of Good Moral Character from where the applicant graduated High School
  5. Certificate of Residency with Recommendation duly signed by the Brgy. Captain from where the applicant permanently resides including a residency in Tangub City, if applicable
  6. Other documentary requirements as per mandated by the Institution for notification

Alfosonsos Student Assistant:

  1. Scholarship Application Form from the Office of Student Affairs
  2. Grade slip of the previously enrolled semester: (Registrar's Office)
    • Office Assistants: GWA 81% with no Failing grades
    • Working Scholar: No failing grade
  3. COR or Study Load from the Registrar's Office
  4. Certificate of Residency with Recommendation duly signed by the Barangay Captain from where the applicant permanently resides including a residency in Tangub City, if applicable
  5. Accomplished ITR or Certificate of Indigency from the BIR/Barangay Certificate and DSWD Certification
  6. Certificate of Good Moral Character from where the applicant graduated High School
  7. Medical Certificate of Good Health from the Guidance and Admission Office
  8. Birth Certificate from the LGU-Tangub City Health Office
  9. Certified True Copy of Form 137/Photocopy of Form 138-Freshmen or Grade Slip from the previous semester if already enrolled in the College from PSA where the applicant graduated High School/ College Registrar
  10. Application Letter addressed to College President from the Applicant
  11. Four (4) pieces 2x2 recent I.D. pictures from the Applicant
  • Sinanduloy Grant, Mr. & Ms. GADTC, Founding Anniversary
  • Student Publication
  • SSC/SBO Grant
  • ROTC Corps Commander
  1. Scholarship Application Form from the Office of Student Affairs/ Supervising Officer/Coordinator
  2. Grade slip of the previously enrolled semester: GWA of 87% in every semester (for Student Publication only) from College Registrar
  3. Certification from the supervising Office/Coordinator from Socio-Cultural Office 
  4. Endorsement from the Scholarship and Welfare Office from OSA-Scholarship and Welfare Office
  5. Brgy. Certification of Residency from the Barangay Office where the applicant resides
Steps Service Details
1 Client Steps: Secure Application Form within the call of application at OSA or download at GADTC Official Website (gadtc.edu.ph).
Agency Action: Provide the Application Form either at the OSA or thru GADTC Official Website.

Client Steps: Submit Accomplished Application Form with all the listed requirements thru the following:

  • Office of Student Affairs or Send to official FB/email stipulated in the form

Agency Action: Receive and check/ evaluate the submitted Application Form and other requirements.

Processing Time: 15 Minutes

2.1 Agency Action: If there are deficiencies, advise the student-applicant to comply.

Processing Time: 10 Minutes

2.2 Agency Action: Process the application of the student- applicant and give the scheduled date for official notification of notice of award
Fees to be Paid: None
Processing Time: 6 Working Days

3 Client Steps: Come back on the scheduled date of the issuance of Notice of Award or wait for the Notice of Award sent thru FB/email.
Agency Action: Issue Notice of Award
