Issuance of Certification of Leave Credit Balance


Certification of Leave Credit Balances is issued by the HRMO to both present and separated officials and employees of Tangub City Government for any of the following purposes:

  1. For reference of the requesting employee;
  2. As supporting documents for loan applications;
  3. As reference of the employees who transferred to other government agencies; and
  4. Other purposes it may be used.

CHECKLIST OF REQUIREMENTS

  1. Filled up Request Form. For present employees, to the Administrative Officer of the office, For separated employees, to the HRMO.

1 Client Steps: Submit the filled up Request Form.​​​​​​​
Agency Action: Receive the filled up Request Form indicating the date and time of receipt and the person who received, advice the client to wait for 10 minutes and immediately start processing the request.​​​​​​​

Processing Time: 10 Minutes
Person Responsible: Administrative Assistant VI (Computer Operator III)
2 Client Steps: Wait for the preparation and issuance of Certification of Leave Credit Balance.​​​​​​​
Agency Action: Issue the requested Certification of Leave Credit Balance and let the requesting party sign the Request Form indicating the date and time he/she received the Certification of Leave Credit Balance.​​​​​​​
