Receive Applications for Published Vacant Positions
Receiving of applications for published vacant positions shall be done within fifteen (15) days from the date of its publication. Applications of all interested applicants shall be accepted during this period subject to pre-screening after the closing date of the publication. Applications may be submitted by personal delivery, by mail, or by email.

CHECKLIST OF REQUIREMENTS
1. Application Letter (1 original/copy of the original) from the applicant.
2. Fully accomplished Personal Data Sheet (PDS) (CS Form No. 212, Revised 2017) with recent passport-sized picture (1 original/copy of the original) can be downloaded at www.csc.gov.ph or from the HRMO.
3. Performance rating in the last rating period, which should be at least Very Satisfactory, shall be required for those who are applying for promotion or transfer (1 original/copy of the original) from the applicant.
4. Certificate of eligibility/rating/license, if applicable (1 photocopy/scanned copy) from the Proper Regional Office of the Civil Service Commission,
Professional Regulation Commission or any agency issuing appropriate eligibility/rating/license required in the published vacant position.
5. Diploma and Transcript of Records (TOR) for the positions whose educational requirement is at least a Bachelor’s degree (1 photocopy/scanned copy) from the college or university where the applicant graduated.
6. Diploma or Form 138 or Certificate of Completion for the positions whose educational requirement is Elementary or High School Graduate or Completion of Vocational/Trade Course (1 photocopy/scanned copy) from the school where the applicant completed elementary, high school or vocational/trade course
