Mental Health Service


The Tangub City Anti-Drug Abuse Team provides assessment, treatment or counselling in a professional relationship to assist an individual or group in alleviating mental or emotional illness, symptoms, conditions or disorders. It includes emotional, psychological, and social well-being. It also helps determine how we handle stress, relate to others, and make choices. This service is given Monday to Friday

CHECKLIST OF REQUIREMENTS

  1. Valid ID (Original) from LGU, LTO, Post Office, PRC and other government agencies
  2. Medical Prescription from a Private or Public Doctor 
Steps Service Details
1 Client Steps:  Proceed to the Mental Health coordinator.
Agency Action:  Let the client sign the Mental health Logbook.
Fees to be Paid:  None 
Processing Time:  1 Hour
Person Responsible:  Mental Health Coordinator
2 Agency Action:  Ask the client about the reason of visit.
  Agency Action:  Take client‘s medical history. 
  Agency Action:  Take client‘s doctors order, medical prescription or referral form ( if no doctors order, medical prescription and referral form proceed directly to 1.4)
  Agency Action:  Take client‘s vital signs and record the data obtained in the Individual Treatment record (ITR)
  Agency Action:  Assess the behaviour, thoughts, feelings and the individual wellbeing of the client.
  Agency Action:  Check the result of the assessment using the Mental  Health Gap Action Plan (mhGAP) of the Department of Health for proper and possible diagnosis and medication to be given.
  Agency Action:  Inform the Physician on duty about the result of the assessment and for the possible diagnosis and medication
  Agency Action: Instruct the client to proceed to the physician on duty
  Client Steps:  Go to the physician on duty​​​​​​​
Agency Action:  Check the assessment result conducted by the mental health coordinator.​​​​​​​​​​​​​​

  Client Steps:  Present the Individual Treatment Record (ITR)​​​​​​​​​​​​​​
Agency Action:  Receive the Individual Treatment Record (ITR)​​​​​​​
  Client Steps:  Get the prescription/referral from the doctor ​​​​​​​​​​​​​​
Agency Action:  Check the possible diagnosis and possible medication to be given to the client.​​​​​​​
  Agency Action:  Prescribe appropriate medicines​​​​​​​
  Agency Action:  If hospitalization or rehabilitation is needed, provide referral
  Agency Action:  If no hospitalization or rehabilitation is needed, advise the client to proceed to the drug counter in charge​​​​​​​
  Client Steps:  Present the prescription  for the release of medicines​​​​​​​​​​​​​​
Agency Action:  At the dispensing area:​​​​​​​​​​​​​​

Processing Time:  15 Minutes

  Client Steps:  Sign the logbook​​​​​​​​​​​​​​
Agency Action:  Get the prescription and release the medicines. Instruct the client for a follow-up session.​​​​​​​

Reengineered systems and procedures: There is no need for clients to inquire about the service available, instead upon arrival of every client, the Public Assistance and Complaints Desk (PACD), will immediately inquire them of the service they expect to avail. Information Billboard and proper sinages will also be put in place.
